Coronary Angiography

What is coronary angiography?

Coronary angiography is an invasive diagnostic examination used to visualize the inside of the arteries that supply blood to the heart (coronary arteries). During the procedure, a thin catheter is inserted into a blood vessel and contrast dye is injected, while X-ray imaging (angiography) captures pictures of the vessels. This allows clear visualization of the internal structure of the coronary arteries and identification of any areas of narrowing or blockage.

Why is it performed?

Coronary angiography is typically performed for the following reasons:

  • To clarify the diagnosis in cases of chest pain (angina) or suspected cardiovascular disease
  • To confirm findings from previous non-invasive tests (ECG, stress test, echocardiography)
  • To determine the degree and location of narrowing or blockage in the coronary arteries
  • To assess the condition of the vessels after a heart attack (myocardial infarction)
  • To evaluate the coronary arteries before surgery (for example, heart valve surgery)

How is it performed?

The procedure is usually carried out in an angiography suite under local anesthesia:

  1. A thin needle is used to access an artery in the wrist (radial access) or groin (femoral access), with the area first numbed using local anesthetic.
  2. A special introducer sheath is placed in the vessel, through which a thin, flexible catheter is guided toward the heart.
  3. Once the catheter reaches the opening of the coronary arteries, contrast dye is injected while X-ray images are recorded.
  4. Images taken from multiple angles give the physician a complete view of the vessels.
  5. After the examination, the catheter is removed and pressure or a closure device is applied to the access site.

The overall duration of the procedure is generally short, though the exact time can vary depending on each patient's clinical situation.

Who might it be recommended for?

Coronary angiography may be recommended for patients with chest pain, shortness of breath, or other cardiovascular symptoms, those with suspicious findings on prior tests, or those considered at higher risk for coronary artery disease. The final decision is made by a cardiologist, taking into account clinical findings, age, coexisting conditions, and the patient's overall health.

Preparation

Before the procedure, the physician typically orders blood tests, an ECG, and other necessary examinations. Patients may be asked to fast for a certain period, inform the doctor of any medications they take (especially blood thinners), and disclose any history of allergic reactions, particularly to contrast dye. Specific preparation instructions are provided individually by the physician.

After the procedure and recovery

Following the examination, the patient is monitored for a period of time to check for bleeding or other complications at the access site. Recovery is usually quicker with radial access compared to femoral access. The physician provides detailed instructions to follow after discharge, including activity restrictions and care of the access site.

Risks

When performed by an experienced team, coronary angiography is generally considered a safe procedure, but like any medical intervention it carries certain risks. These may include bruising or bleeding at the access site, an allergic reaction to the contrast dye, and, rarely, vessel injury, heart rhythm disturbances, or effects on kidney function. The likelihood of these risks depends on the patient's overall health and is discussed individually with the physician before the procedure.
